Change if Personal Details

Please notify the centre of any change in your name, address or telephone number.

This is always useful in case we have to contact you urgently. Please do not delay over this. If you are waiting on a hospital appointment it is important that the hospital should be informed of any change of address

- by telephoning Altnagelvin Hospital, 7134 5171, extension 3446 or 3447

so that your appointment is sent to the correct address.

If your telephone number has changed due to a change in the service provider (eg from BT to Cable), please inform the surgery immediately.

The Practice Computer - How It May Affect You!

Repeat prescriptions, immunisation invitations and other recalls are generated from its data. We ask that you at least acknowledge invitations when you receive them.

Confidentiality is a vital consideration and is preserved under the Data Protection Act.

Again it is important that we are notified of any change of address, otherwise, you may not get these important communications. If you have previously had a test or procedure eg smear or vaccinations, for which you have been invited, please let the receptionist have the appropriate details, so that you will not receive unnecessary reminders and our records can be updated.
